Student Performance

Instructions:
Choose the abilities that most closely represent the student's performance. For any chosen ability, both the grade and % of grade must be selected. The total % must equal 100% and can be checked using a button at the bottom of the form. The calculated "trial average" will be displayed and can be recalculated before you submit the final average (in red).

Important note: The Experiential Education Program requires that students complete an examination and a project or presentation in order for a student to successfully complete her/his rotation. That means that this form will NOT submit a grade for a student if values are not assigned for a project/presentation (Item 23) and for an examination (Item 24).
