NOTES:

1) The Experiential Education Office is implementing a new policy: Preceptors should submit a Mid-rotation Student Evaluation about halfway through the rotation month. The Mid-rotation Evaluation will not be a standard "A-F" grade, but will be an honest assessment of the student's work up to that time using the following scale: "Exceeds Expectations; Meets Expectations; Needs Improvement; Unsatisfactory". The Final Student Grade will be submitted on the standard "A-F" scale as previously.  Note: Effective June, 2006, the Experiential Education Program requires that students be required to do a project or presentation, and complete an examination on the material learned in the rotation. When submitting a final student grade, note that the grading form will not accept the grade submission unless the instructor both assigns a grade for a project/presentation and for an examination, as well as assigning a weighting factor to both these grade requirements.

Before using the spreadsheet, please read the appropriate section of our Frequently Asked Questions page.  Please do not use the spreadsheet or the manual form unless you have called Len Cleavelin at (901) 448-3403, and verified that the form will not work with your browser. This Web based form automatically enters your grade data into a database which we are using to analyze the effectiveness of our professional curriculum.
