Student Performance

Instructions: 
Choose the abilities that most closely represent the student's performance. For any chosen ability, both the grade and % of grade must be selected. The total % must equal 100% and can be checked using a button at the bottom of the form. The calculated "trial average" will be displayed and can be recalculated before you submit the final average (in red). The numerical average will be automatically converted to a PASS or FAIL grade and displayed at the bottom of the form.
