Reid Ware Locus of Control Questionnaire
for Pharmacy Students

 

Dear Student:

As a part of the College of Pharmacy's renewal of its accreditation, the Assessment Committee is gathering information on how pharmacy students learn. Your information is intended to help us improve instruction. We are asking you to assist us in this task by completing this voluntary survey.

This survey is called the "Reid Ware Locus of Control Questionnaire." It requires that you give a response to a series of statements.  You can answer these questions online in about 15 minutes or less. After submitting your results you will be able to review your results and an explanation of them. We do not expect that answering these questions will pose you any personal risks, although it is possible some people might consider the questions to be of a sensitive or of a personal nature.

Your participation in this survey is completely voluntary. If you choose not to participate,  neither your grades nor your class standing will be affected in any way. If you do choose to participate, your answers will be kept confidential, and your answers to the survey will not affect your grades or class standing. If the data we collect in this survey becomes the basis of any papers, publications or presentations, we will use only summary data. No individual survey participant will be identified. You will be asked to supply your UT 9 digit Personal Identification Number (PIN).
